Monday - Thursday 9:00am - 5:00pm

Friday 9:00am - 4:00pm
Closed daily for lunch from 12 noon - 1:00pm

Appointments

To schedule an appointment, please call our office at (801) 298-3247 during regular business hours.

New Patients:
Please arrive 15 minutes prior to your scheduled appointment time to complete registration paperwork. You may download and complete our forms in advance and bring them with you when you come for your appointment.

Cancellations

Please call 24 hours in advance if you must cancel an appointment so that we can open the schedule for other patients who wish to be seen.

Referrals

Some insurances may require a referral for your visit to our office. Please make sure to contact your insurance company and/or your primary care physician to ensure that you get a referral if it is required. Failure to obtain a referral, when necessary, may cause your insurance company to deny your claim.
